Liability/Infectious Disease Disclosure

For myself, my heirs, and my assigns, I hereby release Old Town Animal Care Center (OTACC), its agents, officers, subcontractors, employees, animal parents, customers, and potential customers from any and all liabilities for injuries to myself, my dog, or any other property of mine which arise in any way out of services and/or products provided by or as a consequence of my association with OTACC.  I acknowledge and understand that every dog reacts differently and that animals, by nature, are unpredictable.

Pets are required to have current vaccinations and tests (Rabies, Distemper, Influenza (H3N8 & H3N2) and Fecal within one year, Bordetella within 6 months).  Verification of vaccination history is required prior to leaving the animal in our care, as well as a negative fecal test prior to boarding.  If it is determined that vaccinations are needed or the fecal test is positive, you hereby grant permission to vaccinate and/or deworm your pet.

We cannot guarantee the return of personal items, such as collars, leashes, blankets, and toys. These items are furnished free of charge by Old Town Animal Care Center to all our guests during their stay.  We ask that all personal items are left at home.

In the event of a medical emergency, treatment will be given and a reasonable effort will be made to contact you at the emergency number provided.  Daily monitoring of pets by staff is included in routine care.

I realize that despite the hospital's best efforts, intestinal upsets (vomiting/diarrhea) can occur in boarded animals due to changes in the environment.  If your pet does experience any type of GI (gastrointestinal) upset, our doctors will treat and medicate your pet at during their stay with us.  We will also send medications home if needed.  In addition, we require that all of our guests have the Bordetella booster to prevent kennel cough.  However, despite these efforts, this cough may still develop in some animals. If your pet experiences any of these symptoms after their stay with us, please notify us immediately as most of the time these conditions are not serious and easily treated. Owners are responsible for the cost of any medications or treatments needed.

 

Due to the fact we cannot control the flea and tick population outdoors, we highly recommend a flea and tick treatment 24 hours before boarding or grooming.  If fleas or ticks are found on our guests they will be treated at the owner's expense with both a Flea and a Tick treatment.

 

IF SOMEONE OTHER THAN THE OWNER will be picking up the pet from the hospital, please indicate the name of the person upon check-in.  Please understand that we assume the person who picks up the pet does so with the owner's permission, and therefore we do not require identification.

Dogs may, without warning, bite or cause injury to humans and other dogs.  I acknowledge and understand that there are certain risks involved in participating in social boarding, including, but not limited to dog fights, dog bites to humans or other dogs, and transmission of disease.

Dogs not familiar with OTACC may experience separation anxiety when apart from their humans. Symptoms of separation anxiety include but are not limited to: "kennel nose", soft stool, lethargy, and/or increased thirst after boarding.

Dogs not regularly exposed to play on hard surfaces may experience sore paws, blisters, bruises, or abrasions on the feet (Raw Paw) and may be excessively tired for a few days.

Dogs not regularly socialized may be at higher risk of incidents including, but not limited to: bites, fights, fear aggression, object guarding, behavior problems, and/or acting out.

Excessively long toenails may cause injury.  If at any time your dog's nails are too long, OTACC will trim them at the owner's expense.  Nails should be trimmed regularly to avoid injury.

Clean water is available at all times; however, your dog may still be thirsty after a full day of play.  Be aware of their water intake, as excessive amounts may cause an upset stomach or other problems.

Dramatic changes in food/diet may cause upset stomach, diarrhea, and/or colitis.

Any behavior deemed dangerous or inappropriate by OTACC may result in dismissal from social boarding.

I agree to pay for all services due at the time they are rendered.  I understand any unpaid fees by me will be sent to collections, and I will be responsible for all collection and legal fees incurred by such actions taken.

To the best of my knowledge, my pet is in good health and has not been exposed to communicable diseases (within the last 30 days), internal and/or external parasites.  

I understand that OTACC's staff will give all pets involved in any incident an examination.

I understand that OTACC, to the best of their abilities, will contact me at the phone numbers I have provided should any medical emergency arise.

I will fully cover any and all costs concerning medical attention and I agree to pay for all services due at the time they are rendered.  I understand any unpaid fees by me will be sent to collections, and I will be responsible for all collection and legal fees incurred by such actions taken.

I authorize the use of my dog's visual image(s) and statements in posters, newsletters, or social media sites.
